Please note:

If you HAVE registered your youth in the St. Joseph Youth Ministry program, you only need to fill out the "REGISTERED Permision Form".

If you HAVE NOT registred your youth, you will need to fill out the complete "NON REGISTERED Permission Form". 

Parent's please fill out the Volunteer Driver Waiver, if you have not registered as a Parent Volunteer already.
